Increasing burden of E. coli bacteraemia and changing epidemiology

چکیده
Methods Microbiological records of patients with E. coli bacteraemia were linked with patient data to determine their characteristics, whether CA or HA, and if the E. coli produced ESBL. Blood cultures taken within 2 days of admission were defined as CA, 2 days or more after admission as HA and a new episode if greater than 2 weeks between positive blood cultures. E. coli ESBL was defined by resistance to cefpodoxime, or by resistance to ceftazidime or cefotaxime whilst remaining susceptible to cefoxitin. Where possible, the potential source of the bacteraemia was determined from the antibiotic profile of E.coli isolated concurrently from other specimens.

Increasing incidence of Escherichia coli bacteraemia is driven by an increase in antibiotic-resistant isolates: electronic database study in Oxfordshire 1999-2011.
OBJECTIVES To investigate trends in Escherichia coli resistance, bacteraemia rates and post-bacteraemia outcomes over time. METHODS Trends in E. coli bacteraemia incidence were monitored from January 1999 to June 2011 using an infection surveillance database including microbiological, clinical risk factor, infection severity and outcome data in Oxfordshire, UK, with imported temperature/rainf...
متن کاملClinical significance of healthcare-associated infections in community-onset Escherichia coli bacteraemia.
BACKGROUND The increasing antimicrobial resistance of Escherichia coli infection is of great concern, even for community-onset infections. METHODS We conducted a retrospective cohort study of patients with E. coli bacteraemia who visited the emergency department of the Samsung Medical Center from February 2002 to December 2005 to identify the risk factors for mortality and association between...
متن کاملChanging incidence of detected streptococcal bacteraemia in North Yorkshire, England.
BACKGROUND & OBJECTIVES Streptococcal bacteraemia remains a major and challenging clinical problem throughout the world. The epidemiology of these infections appears to be changing. In the present study we analysed the data collected over a period of 20 yr (1978-1999) to throw light on this. METHODS Laboratory records of all patients yielding clinically significant, non-pneumococcal, nonenter...
متن کاملDescriptive epidemiology of Escherichia coli bacteraemia in England, April 2012 to March 2014
We determined the incidence, risk factors and antimicrobial susceptibility associated with Escherichia coli bacteraemia in England over a 24 month period. Case data were obtained from the national mandatory surveillance database, with susceptibility data linked from LabBase2, a voluntary national microbiology database. Between April 2012 and March 2014, 66,512 E. coli bacteraemia cases were rep...
متن کاملComment on: Impact of antibiotic resistance and of adequate empirical antibiotic treatment in the prognosis of patients with Escherichia coli bacteraemia.
BACKGROUND Escherichia coli is the most frequent Gram-negative organism causing bacteraemia. There are few data about prognostic factors of bloodstream infections due to E. coli. In particular, the consequences of antibiotic resistance and of adequate empirical antibiotic treatment on outcome remain broadly unknown. METHODS We conducted a retrospective cohort study of patients with E. coli ba...
